Your First Visit

After you’ve been greeted by our kind front desk team and all of your paperwork has been filed away, you’ll spend a short time in our waiting room before being escorted back to our treatment area. We’ll ask a series of questions about your oral health concerns, and we’ll perform a full examination of your mouth that will likely include X-rays. We’ll explain if we find anything wrong and go over your treatment options, then schedule follow-up appointments as needed.

Instead of struggling with multiple forms and insurance information while you’re in our waiting room, we instead encourage you to complete the necessary paperwork ahead of time by clicking the links below. In addition to the standard new patient form that will give us the basic information we need in the course of providing dental care, you will also need to complete our COVID questionnaire as part of our current screening process.
